Parent Feedback Survey 2023-2024
Please take a few minutes to fill out the Parent Feedback Survey for the 2023-2024 Season. The Survey is broken into two parts: the first is an evaluation for coaches, assistant coaches and team managers. If you want to fill it out multiple times for the different coaches and assistant coaches, go for it!
The second part relates to general aspects about Brooks Minor Hockey as an organization. You only have to fill this section out once per family.
No question is mandatory so if there's something you can't answer or prefer not to answer, feel free to skip it.
