The BMHA Equipment Request Program helps families facing financial barriers so they can equip their child with needed hockey equipment.
Families wanting to apply for hockey equipment will need to fill out the below form and have it approved by their child's coach. The form will then be sent to the BMHA board for approval.
Upon approval, the recipient will receive information from the BMHA board on the next steps.
